Two Independent Organizations, Two Different Missions

When you see a vehicle described as having a five-star safety rating or an IIHS Top Safety Pick, it's easy to treat those labels as equivalent stamps of approval. They aren't. The National Highway Traffic Safety Administration (NHTSA) is a federal government agency within the U.S. Department of Transportation. The Insurance Institute for Highway Safety (IIHS) is a nonprofit research organization funded by auto insurers. Both evaluate vehicle crashworthiness, but they were created with different goals, use different test methods, and produce different types of results.

Understanding what each system actually measures helps you compare vehicles on safety grounds without relying on marketing shorthand. It's also useful context when you're evaluating a vehicle before purchase, since safety ratings alone don't capture everything a test drive can reveal.

How NHTSA's New Car Assessment Program Works

NHTSA's New Car Assessment Program (NCAP) assigns vehicles one to five stars across three categories: frontal crash, side crash, and rollover resistance. Stars are awarded separately for each category and for driver and passenger positions, then combined into an overall rating. Five stars in the overall rating reflects the highest performance across those tests.

NHTSA's frontal crash test involves a full-width barrier impact at 35 mph — meaning the entire front of the vehicle strikes the barrier simultaneously. Side tests include both a moving deformable barrier (simulating another vehicle) and a pole test. Rollover resistance is calculated using a static stability factor and, for most vehicles, a dynamic driving test.

One important limitation: NHTSA star ratings are primarily about occupant protection inside the vehicle during specific crash types. They do not directly score pedestrian safety or headlight effectiveness, and they were not originally designed to evaluate some of the more complex real-world crash geometries that modern testing has introduced.

How IIHS Ratings and Awards Work

IIHS uses a four-tier rating scale — Good, Acceptable, Marginal, or Poor — for each of its individual tests. Vehicles that meet a defined combination of scores earn either a Top Safety Pick (TSP) or the higher designation Top Safety Pick+ (TSP+). These award thresholds are updated periodically as IIHS raises its standards.

IIHS tests include a moderate overlap frontal crash (40% of the vehicle's width strikes a barrier), a small overlap frontal crash (25% overlap on the driver's side), a side crash with a taller barrier that better represents SUV-to-car impacts, and a roof strength test. Critically, IIHS also evaluates headlights on a Good-to-Poor scale — a factor with real safety relevance that NHTSA's star rating does not incorporate.

For TSP+, vehicles must also demonstrate adequate front crash prevention capability, tested through vehicle-to-vehicle and vehicle-to-pedestrian scenarios. These requirements link directly to the kind of active safety systems modern vehicles increasingly carry.

Where the Two Systems Diverge Most

The most significant divergence is the small overlap frontal test. In a real-world crash where only the corner of the vehicle strikes another object — a telephone pole, a tree, or another car at an angle — very little of the vehicle's front structure is engaged. IIHS introduced this test because many vehicles that performed well in full-width or moderate-overlap crashes showed significant structural failure in small-overlap scenarios. NHTSA's current NCAP protocol does not include a small overlap test, though the agency has discussed updates to its program.

Headlight ratings are another meaningful gap. IIHS has rated thousands of vehicle headlight configurations and found wide variation even within the same model, depending on trim level and optional packages. A vehicle with poor-rated headlights reduces a driver's ability to detect hazards at night — a real safety gap that stars alone won't reveal.

Neither system is a complete substitute for the other. A vehicle can earn five NHTSA stars and still receive a Marginal rating in an IIHS small overlap test. The reverse is also possible. If you're already paying attention to your vehicle's systems, it's worth applying the same attention to the safety data before you buy.

How to Use Both Ratings Together

The practical approach is straightforward: look up a vehicle on both the NHTSA website (nhtsa.gov) and the IIHS website (iihs.org) before forming a safety judgment. Both sites are free and publicly accessible. On IIHS, pay close attention to the small overlap rating, the side crash result, and the headlight rating — these are where vehicles most frequently fall short even when overall marketing language sounds positive.

On NHTSA, review the category-level stars rather than just the overall rating. A five-star overall score can sometimes mask a lower score in a specific category such as rollover or rear-seat side protection.

Keep in mind that both programs test vehicles as delivered from the manufacturer. Aftermarket modifications, added weight, or changes to suspension and tires may affect real-world crash behavior in ways neither rating system accounts for. Safety ratings are a useful and well-validated tool for comparison — but they represent standardized lab conditions, not every scenario a driver will encounter.
